Department of Transportation (DOT) Substance Abuse Professional (SAP) Program

Our mission is to provide substance use disorder counseling, treatment, family support, and recovery resources to individuals who cannot afford care. We serve Medi-Cal recipients, people whose insurance does not cover the necessary treatment, formerly incarcerated nonviolent offenders, parolees, and other underserved members of the community.

We also provide referrals for transitional housing and help individuals navigate the placement process. Working alongside Dedicato Treatment Center, our compassionate professionals bring more than 20 years of experience helping individuals and families affected by substance use disorders.

What this page is for: This program is for employees in DOT safety-sensitive positions — including CDL drivers, pilots, railroad workers, transit operators, pipeline workers, and others — who have violated DOT drug and alcohol regulations and need to complete the federal return-to-duty process.

Who Needs a DOT SAP Evaluation?

You are required to complete the DOT SAP process after:

  • A positive DOT drug test (marijuana, cocaine, amphetamines, opioids, PCP)
  • A positive DOT alcohol test (0.04 or greater)
  • A refusal to test (including adulterated / substituted specimen)
  • Other DOT violations reported to FMCSA Clearinghouse

Important: Once a violation is reported, you are immediately removed from safety-sensitive functions and cannot return until the full RTD process is completed.

What is a DOT Substance Abuse Professional (SAP)?

The SAP is a person who, per 49 CFR Part 40, evaluates employees who have violated a DOT drug and alcohol program regulation and makes recommendations concerning education, treatment, follow-up testing, and aftercare.

SAP is a Gatekeeper for Safety

SAP does not advocate for employer or employee. The SAP’s first duty is to protect public safety by ensuring only qualified, rehabilitated individuals return to safety-sensitive work like operating trucks, buses, trains, aircraft, oil tankers, and pipelines.

Qualified & Credentialed

Our SAP meets all DOT requirements: Licensed/certified professional with DOT-approved SAP qualification training, passing SAP exam, and 12-hour continuing education every 3 years. [Dr. Marshall: MAC, SUDCC-IV-CS, LAADC, etc.]

The Two Phases of the DOT SAP Return-to-Duty Process

Phase 1: Return-to-Duty (RTD) - To Become Eligible to Drive/Work Again

Initial SAP Evaluation

Face-to-face clinical assessment (in-person or virtual per DOT rules). Review of violation, substance history, work history, medical/mental health.

Education / Treatment

SAP recommends appropriate level: 8-hour drug education, outpatient, IOP, etc. We refer to approved providers and monitor progress.

Follow-Up SAP Evaluation

Second face-to-face to determine compliance with recommendations. If compliant, SAP provides RTD compliance report.

Return-to-Duty Test

Employer orders observed DOT return-to-duty drug & alcohol test. Negative result is reported to Clearinghouse. Only then can you resume safety-sensitive duties.

How Long Does It Take? How Much Does It Cost?

Timeline:
No fixed timeline per DOT. Phase 1 typically few weeks to several months depending on treatment recommended and how fast you complete it. Phase 2 is 12-60 months of follow-up testing.

How to Designate Your SAP in FMCSA Clearinghouse

  1. Log in to FMCSA Clearinghouse via login.gov
  2. Go to Driver Dashboard → “Designate a Substance Abuse Professional”
  3. Search and select your SAP and submit request
  4. SAP accepts designation – you generally cannot switch SAP after acceptance
